AirOntario flight 1363, an F-28 with 65 passengers and four crew, crashed during take-off from Dryden airport on 10 March 1989. CASB investigation and enquiry pointed to crew communications, corporate pressure, ground resources, fatigue as causes. The Commission of Enquiry led by Justice Moshansky reported *significant failures, most of them beyond the captain's control, that had an operational impact.* The video recreates the accident and examines the causal factors unearthed during investigation. Excellent case study. The Transport Canada Progressive Literature Review quotes $350 from Brent Hayward, Quantas HF Manager and recommends for commercial, instrument and airline pilot training. After returning to Riyad and landing the aircraft had a protracted ground roll and flight crew failed to initiate emergency procedures preventing ground-based emergency services action which might have controlled the cabin fire.
